Omegaven, omega-3, 간기능 개선에 대한 효과

미국에서, PN받는 환자에게서 지질 측면의 영양소를 채워주기 위해 soybean oil(LCT)가 함유된 lipid 제형이 선택된다.

지질은 같은 g당 고함량 칼로리를 채워주고 필수 지방산 섭취를 위해 필수적이다.

 

하지만 지질이 PN associated liver disease를 유발한다고 알려져 있다.(특히 소아에게서) 

Phytosterols(soybean oils에 함유된)이 담즙 배설에 나쁜 영향을 미친다고 생각된다.

omega-3 fatty acid가 함유된 제품(Omegaven™)이 실제 steatotic liver injury에 유용할지에 대한 연구가 진행 중에 있다.

이 study에서의 가정은

(1) inhibition of de novo lipogenesis

(2) reduction of arachidonic acid-derived inflammatory mediators

(3) prevention of essential fatty acid deficiency through the presence of small amounts of arachidonic acid

(4) improved clearance of lipids from the serum

 

- FDA insert paper상 omega-3/omega-6 비율이 clinical outcome에 영향을 미치는지는 아직 밝혀지지 않았다고 함.

- animal study에서 bile flow를 호전시킨다는 연구 결과가 있음.

Omegaven™

- 함량 : 100 mg/mL(10% lipid 제형, 일반적으로 SMOF lipid의 경우 20 %)

- indication : a source of calories and fatty acids in pediatric patients with parenteral nutrition-associated cholestasis (PNAC). 이때 PNAC 예방요법으로의 indication은 없다.

** direct(conjugated) bilirubin 수치가 2 mg/dL 이상시 투약, 수치 정상화시 stop 고려(다른 lipid로 변경 고려)

- administration : peripheral or centeral (둘 다 가능)

- dosage : The recommended daily dose (and the maximum dose) in pediatric patients is 1 g/kg/day.

- rate : 

▶ 초기 : not exceed 0.05 mL/minute for the first 15 to 30 minutes of infusion

▶If tolerated, 30분마다 점진적 증량, The maximum infusion rate should not exceed 1.5 mL/kg/hour (=0.15 g/kg/hour)

▶ plasma TG 모니터링 필수

 hyperTG 환자에게서는 감량 고려될 수 있음 (0.5-0.75 g/kg/day)

- contraindication : hyperTG (1,000 mg/dL 이상) ; SMOF lipid의 경우에도 그러하다.

** Omegaven 투여 중 hypertriglyceridemia 발생시 (triglycerides greater than 250 mg/dL in neonates and infants or greater than 400 mg/dL in older children) 4시간 동안 infusion HOLD, 이후 정상 TG lab 확인될 때까지 주기적인 check, 결과에 따라 재시작을 고려

- drug interaction : antiplatelet agent, anticoagulatns (해당 약제와 oral 오메가-3 병용 환자에게서 prolonged bleeding time이 관찰됨)
