ASPEN lipid injectable emulsion safety; recommendations for adult patients

 

과거 ILE(injectable lipid emulsion)은 soybean oil 100% 였지만 대체 ILE이 허가되면서 soybean 비율이 낮아졌다.

 

 

• With reduced soybean oil products, ILE may be provided to critically ill adults upon PN initiation without producing the inflammatory and immunosuppressive effects associated with SO-ILE.
• Adult minimum EFA recommendations are at least 2%-4% of energy form LA and 0.25%-0.5% of energy from ALA.
• Dosing for maintenance or long-term therapy should be about 1 g/kg/day in the stable adult patient.
• ILEs that are less than 100% soybean oil require a higher minimum dose vs SO-ILE due to lower LA and ALA content.
• The minimum dose to prevent ERAD is not the daily recommended dose.
• EFA content may vary within a preestablished range with may account for variations of reported amounts in the literature.
• These are minimum estimated dosing recommendations; indiciduals may require higher ILE dosing to prevent or treat EFAD depending on clinical circumstances.
• Greater than ILE minimum dosing allows for a reduction in the amount of dextrose needed to provide appropriate energy intake.

 

 

필수지방산 결핍 예방(essential fatty acid deficiency (EFAD))을 위해 2013 ASPEN에서 다음과 같이 소개하고 있다.

2주 이상 입원하여 PN을 공급받는 환자는 필수 지방산 결핍을 예방하기 위해 100 g/wk의 지방을 공급받아야 한다. EFAD의 증상 징후는 건조, 비닐성 발진, 탈모, 혈소판 감소, 빈혈, 상처 치유 지연 등이 있다.

 

2015 Canadian Clinical Practice Guidelines에서는 장내 영양이 가능하거나 PN 유지가 짧을 것으로 예상되는(10일 이내) 환자에서 첫 10일 동안 SO-ILE를 미룰 것을 추천한다.(recommend withholding the addition of SO-ILE for the first 10 days)

 

ASPEN/Society of Critical Care Medicine (SCCM) guideline에서는 PN 치료 첫 주동안은 SO-ILE을 미룰 것을 추천한다. EFAD 고위험 환자의 경우에는 SO-ILE을 보류하기보다는 100 g/wk로 제한해볼 수 있다.

 

2018년 ESPEN에서 발표한 지질에 대한 권고사항에서는 안정적인 중환자에서 PN therapy 시 ILE은 1.5 g/kg/day(oil source에 상관없이) or 0.1-0.2 g/kg/day of FO-ILE+SO-ILE 복합제제는 PN 시 필수적이라 권장했다.

 모니터링

- TG(triglyceride)를 기저치와 주 1회 f/u을 권장함

- ILE 투여 중 TG> 400 mg/dL 된다면 중단 혹은 제한. 하지만 EFAD 위험이 있기 때문에 2주 이상의 중단을 권장하지 않음
