NHSN: National Healthcare Safety Network;

SSI: surgical site infection;

OX/METH: oxacillin/methicillin;

ESC4: extended spectrum(ES) cephalosporins (cefepime, cefotaxime, ceftazidime, ceftriaxone);

FQ3: fluoroquinolones (ciprofloxacin, levofloxacin, moxifloxacin);

MDR1: multidrug resistance 1 gene (pathogen must test as “I” [intermediate] or “R” [resistant] to at least one drug in 3 of the 5 following classes: ESC4, FQ3, aminoglycosides, carbapenems*, and piperacillin [PIP] or piperacillin/tazobactam [PIP/TAZ]);

NA: not available;

VAN: vancomycin;

AMINOS: aminoglycosides (amikacin, gentamicin, tobramycin);

ESC2: ES cephalosporins (cefepime, ceftazidime);

MDR2: multidrug resistance 2 gene (pathogen must test as I or R to at least 1 drug in 3 of the 5 following classes: ESC2, FQ2, AMINOS, carbapenems, and PIP or PIP/TAZ.

* Carbapenems are imipenem and meropenem.
