Case.1 5-FU 1200 mg over 44 hrs?

일반적으로 위장관계 암에서 사용되는 FOLFIRI regimen은 다음과 같다.

¶ A lower initial starting dose of irinotecan may be considered for patients with poor performance status, prior pelvic or abdominal radiotherapy, or increased bilirubin levels. Whether a reduced starting dose is needed in patients who are homozygous for the UGT 1A1*28 allele (Gilbert syndrome) and whether testing for this allele should be carried out prior to starting irinotecan is controversial. Refer to UpToDate topic on "Enterotoxicity of chemotherapeutic agents".
Δ Diluent solutions should not be modified without consulting a detailed reference due to potential incompatibility(ies).
◊ Leucovorin dose is given for d,l-racemic mixture. Use half the dose for LEVOleucovorin (l-leucovorin).
§ At many institutions, the day one bolus dose of FU is routinely omitted, starting with cycle 1, to improve tolerability in the setting of metastatic disease.
¥ If there is no grade 1 or worse toxicity 1 in cycles 1 and 2, some clinicians increase the dose to 3000 mg/m2 starting with cycle 3.

 

센터에 따라 다르겠지만 우리 병원에서는 5-FU의 경우 BSA당 2400 mg을 46hr 시간에 걸쳐 투여하거나 BSA당 1,200 mg씩 23hr에 걸쳐 각각 조제한다.

 

Neuroendocrine carcinoma of nasopharynx 환자가 FOLFIRI regimen을 사용하였는데 

• Irinotecan  180 mg/m2 IV over 90 min D1 q2wks
• Leucovorin 400 mg/m2 D1
• 5-FU  400 mg/m2 bolus D1
• 5-FU  1200 mg/m2 IV over 44 hrs D1-2

허가초과 항암요법이란?

식품의약품아전처장의 평가를 거쳐 정해진 허가사항과 달리 안전성, 우효성이 불명확하므로, "허가초과 항암요법"을 사용하고자 하는 경우는 최소한의 안전성, 유효성을 확보하고자 승인 절차를 거쳐 사용하도록 하고 있다.

사전승인과 사후승인으로 나뉘며 건강보험심사평가원 사이트에서 이미 인정된 항암 regimen을 확인할 수 있다.

절차에 따라 허가초과 항암요법 사용 시 약제 비용은 환자 전액본인부담(100/100)이다.

신경내분비암

투여대상: etoposide + platinum 항암요법에 실패한 신경내분비암(grade 3)

투여단계: 2차 이상

투여요법: 고식적요법(palliative)

 

• 2주 마다
• irinotecan 180mg/m2 D1
• leucovorin 400mg/m2 (2hr infusion) D1
• 5-FU 400mg/m2 (10min bolus) D1
• 5-FU 1200mg/m2 (44hr infusion) D1-D2

*일반적으로 FOLFIRI regimen의 경우 5-FU 1200 mg/m2 over 23 hrs D1-2인데 반해 신경내분비암의 경우 44hrs에 걸쳐 D1-2로 투여됨 (total 5-FU 1200 mg/m2로 continuous infusion)
